Welcome to MYC® 2024-2025

Please initial each section ____, sign/date at the end, and return this form to the teacher at the first lesson.
Classes begin the week of September 5th

____ TUITION FEE:

36 lessons- include all classes and recitals. Cheques are due at the first lesson. Please make all cheques payable to Susan Nokes. 9 monthly post dated cheques OR Recurring Etransfer

□ Regular- $104 Post-dated Sept. 1st – May 1st note: ________________________

NSF charge of $25 will apply.

____ CLASS SCHEDULES/Cancellations:

Classes meet once a week for one hour each week with a parent or designated music partner. Students are grouped according age and level.   Class times may be subject to change with appropriate notice.

____ ARRIVAL/PUNCTUALITY:

Please come to class prepared and on time to avoid disruption to others! Hooks are provided for coats, indoor shoes or socks only in the studio.

____ ABSENCE:

In the event of unavoidable absence, please let me know. Missed lessons will not be refunded; however, I will phone or e-mail you the homework. Student may participate online when in-person is not available.

All Covid safety protocols will be in place, including screening before attending in-person lessons.

____ WITHDRAWAL:

Every student stops taking lesson at some point.  Ideally, this decision is made thoughtfully with plenty of discussion between the teacher, parent, and student so that lessons can cease with a positive sense of closure.  For this reason and since your spot cannot be filled once lessons begin, the teacher asks  for two months notice prior to discontinuing lessons.  Note: At the discretion of the teacher, students may be asked to discontinue lessons at any time if they have unpaid tuition, an excess amount of   absences/unprepared lessons, or if they show a lack of interest.

____ PARENT PARTICIPATION:

The parent’s role is to provide support to your child. You need to keep your child organized and be a positive example as you also participate, listen and learn. Please help by finding manual pages, tin sheet, magnets and rhythm bag during the lesson to keep the class running efficiently. Please no cell phones during lessons.

____ SIBLINGS:

Other siblings require childcare if not enrolled in class, unless an unforeseeable need arises or planned in advance.

____ PRACTICE EXPECTATIONS:

You will learn a variety of music concepts throughout the course. It is essential to practice all the elements when practicing at home. We all learn best with review. Try to start and end practices with something fun. Schedule practices during the day when your child is at their best. Five days of practice each week is expected and is rewarded with a sticker. Treasure Chest is a reward practicing and is filled by donations from parents. I also reward a ‘perfect practice record’ with a medal at the year-end Recital. Keys in activity book must be colored before you come to class.

____ COMMITMENT:

Beginning music lessons is a commitment to your child’s music education. It is not just one hour of class time each week but also consistent practice time at home. Short practices a little each day work best. There will be days when your child will not want to practice, this is normal and does not mean that music lessons  should come to an end.

____ COMMUNICATION:

Your input and communication is very important to me. You may call, e-mail or text me anytime if you have questions or concerns.

I agree to allow  photographs or videos of my child  to be included in future MYC® related publications; eg. MYC® webpages /Facebook  yes□   no□
